      Certainly, but the Malkavians who last to become Elders or Methesulahs have very specific Derangements. The ones with suicidal tendencies don't make it past 50 let alone 500, and the ones who flip out and attack people who use puns (bad example but you get the idea) don't fare too well. The long term survivors are the ones with the broken Megolomania, which is a Merit with a downside you can ignore for one point of Willpower.

      RAW, all Malkavian Derangements are ignorable for a scene with Willpower... so... yeah.

      And split personality doesn't have to be a problem. It can be a good thing, even, since they can run separate morality tracks (there's at least one canon example of that) and that gives you flexibility (even if you're in an inner turmoil, but you win some you lose some).
