What does it have in terms of crunch? For instance, are there any denizens that are found primarily in a given realm?

Not much. A few Birthrights and benefits scattered through the book, and a Mechanics chapter at the end with Boons, specialized Purviews, Origin Paths, the Psychopomp Calling, and Relics. By and large, this is mainly a setting book.

So is this the first dedicated setting book for Scion 2E? Curious because I don't recall seeing anything that actually addresses how the whole "all myths are real" thing has changed the mundane world, but I might have missed something.

I read, for now, only Atlantis section. It's really 'SPA health resort, coming from Plato's Atlantis' - and cross Theoi/Netjer nexus place. It's... interesting - but totally not I would imagine thinking 'Atlantis in Scion'. It takes a while pointing it's THIRD Atlantis variant in gameline - first in Origin as 'underwater fallen kingdom' and second being 'cosmic lost continent with space gods' of Mysteries of the World. It get's some timeline events how this 'Atlantis as SPA' things shapen it. So, yeah, it at least in this examples have a small example how "All Myths Are Real" in it's history.

They have pretty much left that up for each table to define. That is what the first chapter of Mysteries of the World goes into with the different myth levels and genres.
