Just putting this here as a first draft / pitch for a character of mine. Too Solid Flesh and Shared Burden being things sort of remove the need for an Oneirophysic's old benefits.
Oneirophysic Privileges
Hippocratic Oath: "Never do harm" is the oldest and most sacred oath of the medical field, and The Order of Oneirophysics are no different. The Order no longer formally pledges their members to act responsibly and never use their medical talents to do harm, but it is still one of their core tenants. An Oneirophysic who uses Dream Triage to harm someone or an Oneriophysic that otherwise abuses a caregiver relationship after agreeing to serve in that role undermines their identity. Up to once per scene this can result in a breaking point roll with a -3 penalty.
Emergency Care: Oneirophysics are tasked with traveling the world and treating all who seek their aid. A wise doctor beware, however, lest she succumb to the cause of her patient's misery. This is an approach that allows a Huntsmen to gain a point of yearning when they use someone who is injured or otherwise in need of the Oneirophysic's help as bait for a trap.
Dream Triage: An Oneirophysic can have a talent for affecting a person's body through their dreams, allowing them to use the Medicine skill to treat and diagnose a person normally while inside their Bastion. A simple change to the narrative of the dream can alleviate any requirements for particular tools or an appropriate setting. The patient's mind shifts to accept the Oneirophysic's role of being a doctor, allowing the them to add their Wyrd to all rolls to diagnose or treat a patient while in that patient's Bastion. As an instant action, an Oneirophysic may use a (Dexterity or Intelligence) + Medicine + Wyrd roll to apply the benefits of Healing Sleep or Psychotherapy to someone even when the Oenirophysic isn't dreamscaping. By spending a point of Glamour per roll while physically touching a sleeping patient, all of the above effects can be used without actually entering the target's dreams, but in that case the patient's Bastion rating serves as a penalty to all of the rolls.
Medical arts can be used to harm as well as heal, though the order officially trains each of their members to do the least amount of harm possible. An Oneirophysic may perform an environmental attack using (Intelligence or Manipulation) + Medicine + Wyrd against someone to afflict someone with all sorts of terrible malpractice. On an exceptional success, the Oneirophysic may apply a sensible physical condition such as blindness or arm wrack. This condition becomes Persistent and grants a beat in an scene where the penalty hinders the character and is resolved by being subject to any form of healing magic that succeeds on a Clash of Wills or if the Oneirophysic that applied it dies. The Oneirophysic may remove the condition without resolving it at any time.
Status [Oneirophysic]: Oneirophysics are a network of professionals that bears a strong resemblance to a guild. As such, an Oneirophysic may purchase dots in Status relating to this entitlement. While the primary benefit is getting access to the Order's resources and collaboration, a character's dots in Status also determines how much Aetherwine they can have access to.
* 0 Initiates: Those who have just learned or naturally developed the capacity for Dream Triage. It is hypothetically possible that someone develops the Oneirophysic's powers without being trained in the Hippocratic Oath if they have no Status, but the Order aggressively seeks out such anomalies to make sure that they join.
* 1-2 Interns: At this rank, Oneirophysics are servants to their more experienced brethren. They practice their healing arts only with permission or supervision and at this stage are considered students.
* 3-4 Residents: Those who have achieved this rank are released from service and expected to serve Changeling Society through medicine independently. They may or may not have their own Interns.
* 5 Physic: The leaders of the Order who set policy and arrange meetings.
Aetherwine: This is a special sedative distilled from particular goblin fruits that can instantly send someone into a dreaming sleep. It is inert until a Changeling reflexively spends a point of Glamour to catalyze the cocktail into something useful. Any time after that within the scene, the Aetherwine can be drunk or applied to a cloth and held over a person's mouth and nose (requiring a move performed within a grapple) to take effect. The patient must succeed in a (Stamina + Resolve - Changeling's Wyrd) roll if they wish to stay awake. If they fail that roll or choose to succumb to the effects, then they immediately enter a dreaming sleep. An Oneirophysic may only have an amount of doses of Aetherwine at a time equal to their dots in Status [Oneirophysic].
