    Just putting this here as a first draft / pitch for a character of mine. Too Solid Flesh and Shared Burden being things sort of remove the need for an Oneirophysic's old benefits.

    Oneirophysic Privileges
    Hippocratic Oath: "Never do harm" is the oldest and most sacred oath of the medical field, and The Order of Oneirophysics are no different. The Order no longer formally pledges their members to act responsibly and never use their medical talents to do harm, but it is still one of their core tenants. An Oneirophysic who uses Dream Triage to harm someone or an Oneriophysic that otherwise abuses a caregiver relationship after agreeing to serve in that role undermines their identity. Up to once per scene this can result in a breaking point roll with a -3 penalty.

    Emergency Care: Oneirophysics are tasked with traveling the world and treating all who seek their aid. A wise doctor beware, however, lest she succumb to the cause of her patient's misery. This is an approach that allows a Huntsmen to gain a point of yearning when they use someone who is injured or otherwise in need of the Oneirophysic's help as bait for a trap.

    Dream Triage: An Oneirophysic can have a talent for affecting a person's body through their dreams, allowing them to use the Medicine skill to treat and diagnose a person normally while inside their Bastion. A simple change to the narrative of the dream can alleviate any requirements for particular tools or an appropriate setting. The patient's mind shifts to accept the Oneirophysic's role of being a doctor, allowing the them to add their Wyrd to all rolls to diagnose or treat a patient while in that patient's Bastion. As an instant action, an Oneirophysic may use a (Dexterity or Intelligence) + Medicine + Wyrd roll to apply the benefits of Healing Sleep or Psychotherapy to someone even when the Oenirophysic isn't dreamscaping. By spending a point of Glamour per roll while physically touching a sleeping patient, all of the above effects can be used without actually entering the target's dreams, but in that case the patient's Bastion rating serves as a penalty to all of the rolls.

    Medical arts can be used to harm as well as heal, though the order officially trains each of their members to do the least amount of harm possible. An Oneirophysic may perform an environmental attack using (Intelligence or Manipulation) + Medicine + Wyrd against someone to afflict someone with all sorts of terrible malpractice. On an exceptional success, the Oneirophysic may apply a sensible physical condition such as blindness or arm wrack. This condition becomes Persistent and grants a beat in an scene where the penalty hinders the character and is resolved by being subject to any form of healing magic that succeeds on a Clash of Wills or if the Oneirophysic that applied it dies. The Oneirophysic may remove the condition without resolving it at any time.

    Status [Oneirophysic]: Oneirophysics are a network of professionals that bears a strong resemblance to a guild. As such, an Oneirophysic may purchase dots in Status relating to this entitlement. While the primary benefit is getting access to the Order's resources and collaboration, a character's dots in Status also determines how much Aetherwine they can have access to.
    * 0 Initiates: Those who have just learned or naturally developed the capacity for Dream Triage. It is hypothetically possible that someone develops the Oneirophysic's powers without being trained in the Hippocratic Oath if they have no Status, but the Order aggressively seeks out such anomalies to make sure that they join.
    * 1-2 Interns: At this rank, Oneirophysics are servants to their more experienced brethren. They practice their healing arts only with permission or supervision and at this stage are considered students.
    * 3-4 Residents: Those who have achieved this rank are released from service and expected to serve Changeling Society through medicine independently. They may or may not have their own Interns.
    * 5 Physic: The leaders of the Order who set policy and arrange meetings.

    Aetherwine: This is a special sedative distilled from particular goblin fruits that can instantly send someone into a dreaming sleep. It is inert until a Changeling reflexively spends a point of Glamour to catalyze the cocktail into something useful. Any time after that within the scene, the Aetherwine can be drunk or applied to a cloth and held over a person's mouth and nose (requiring a move performed within a grapple) to take effect. The patient must succeed in a (Stamina + Resolve - Changeling's Wyrd) roll if they wish to stay awake. If they fail that roll or choose to succumb to the effects, then they immediately enter a dreaming sleep. An Oneirophysic may only have an amount of doses of Aetherwine at a time equal to their dots in Status [Oneirophysic].

      Adjusting this based on feedback. I feel like each Entitlement should have an additional breaking point, an approach that has to be fulfilled before any others, a unique benefit, an item (usually a token, in this case a special Goblin Fruit thing) and a favored contract. I took The Plunge and made it into a general merit any Changeling can take because it doesn't feel like an Oneirophysic specific thing.

      MERITS
      These merits are generally available.

      The Plunge (**)
      Sometimes a dreamer can't wait for a Changeling to find them in the Hedge. Sometimes you are needed right now and all you can do is throw all of your magic into someone's dream right now. A Changeling with this merit can perform the following actions on a dreamer by touching the outside of their Bastion or by touching their sleeping body without actually entering the target's dreams: Bolster Intensity, Scour Intensity, Dream Riding (all forms), and Dreamscaping (all forms). Any rolls to perform these actions take a penalty equal to the Bastion rating you are working through and you must spend a point of willpower to even make the attempt. Oneiromachy, Dream Weaving, and Contracts may not be performed through The Plunge.

      Privileges

When Dreamwalking, any use of the Medicine skill to diagnose or treat the dreamer applies to their physical body as though the Oneirophysic had physically performed those actions. A dream can be altered to be more suitable for medical purposes (a suitable location, better equipment, et cetera), effectively resulting in an equipment bonus on any mundane medicine rolls performed in the dream and allowing the interval for those rolls to be one per Instant Action. Making the dream suitable requires a Wits + Medicine + Wyrd - Desired Equipment Bonus roll as an instant action. Dream Triage functions normally when using The Plunge.

      Aetherwine: This functions as presented in the book with the following changes: although aetherwine works against any living creature, it counts as a Goblin Fruit; Oneirophysics can either craft their own Aetherwine using the rules provided or get up to their Medicine Dots in free Aetherwine per month from other members of the Entitlement; and the sleep created b aetherwine counts as a fallow dream for the entire duration of the sleep.

      Favored Contract (Shared Burden): When using Shared Burden on a sleeping patient or on anyone while the Oneirophysic is in a dream, the user may spend 1 willpower to heal the patient's rightmost health box before applying the Contract's effects. In addition, the Oneirophysic may select to transfer a condition afflicting the patient to themselves in place of healing any additional health levels. The patient is not considered to have resolved the condition.

        Adjusted based on feedback again. Wording all over has been polished and streamlined. The Plunge now costs 1 Willpower to make the connection instead of one willpower per roll. Splitting things up to "Responsibilities," which includes the breaking point and a brand new Entitlement Taboo and "Privileges" which are the advantages. Shared Burden now only has the Condition transfer because the rightmost healthbox thing felt way too much like two favored bonuses stapled together, which looked clunky. Between the two effects, the Condition transfer felt like better story. The "preparing the dream" aspect of Dream Triage was made passive to cut down on rolling and I explained what exactly happens to the patient who undergoes dream triage's body since that was causing some confusion.

        The Plunge (••)
        Prerequisites:
        Changeling
        Some Changelings cultivate the ability to alter dreams by touching the sleeper in the waking world instead of journeying through the Hedge. Forming this connection requires skin to skin contact and the expenditure of one willpower, which allows the Changeling to know what is going on in the sleeper's dream until the Changeling stops touching them. From there, the Changeling can Bolster Intensity, Scour Intensity, use Dream Riding and Dreamscaping on the dream normally, although each roll to do so is penalized by the sleeper's Bastion rating. Oneiromachy, Dream Weaving, and Contracts may not be performed through The Plunge.

        Responsibilities

        Hippocratic Oath: An Oneirophysic who abuses their caregiver status after agreeing to serve in that role undermines their identity, including any deliberate use of this Entitlement's Privileges to harm someone. Up to once per scene this can result in a breaking point roll with a -3 penalty.

        Call to Action: As a Minor Taboo, an Oneirophysic will supply emergency medical aid to anyone who asks for it and needs it without regards to their station, allegiances, or payment.

        Privileges

        Entitlement Approach (Emergency Care): The burden of a doctor is the willingness to expose themselves to the dangers afflicting their patients and pull success from a blighted future. Far worse then leprosy lurks in the dreams that Oneirophysics plunge in to. This is an approach that allows a Huntsmen to gain a point of yearning when they use someone who is seemingly injured or otherwise in need of the Oneirophysic's help as bait for a trap. A Huntsmen must fulfill this approach before benefiting from the Changeling's Court Approach or the Approach gained through high mantle.

        Favored Contract (Shared Burden): When using Shared Burden on a sleeping patient or on anyone while the Oneirophysic is in a dream, they may spend one Willpower to transfer a condition afflicting the patient to themselves in place of healing any health levels. The patient is not considered to have resolved the condition.

        Dream Triage: Oneirophysics may use Medicine Skill in place of Expression Skill when engaged in Oneiromachy or Dreamscaping. While Dreamwalking, an Oneirophysic may diagnose and treat the dreamer using the Medicine skill despite not being physically present. The patient's body mends itself through an augmented psychosomatic process and Baubles in the form the stitches, splints, or other medical devices necessary for treatment (and last until that treatment is complete) form. All mundane medicine rolls performed inside of a dream gain an equipment bonus of the Oneirophysic's Wyrd as the dream manifests tools and conditions appropriate to the task. Mundane medical rolls performed in a dream may also be done with a time interval of an Instant Action regardless of how long it normally takes. Dream Triage functions normally when using The Plunge.
